Diabetes Management Program

Many times, we all hear stories about adults and senior citizens who, simply because they are unable to manage their insulin care, are left with no choice other than placement in skilled nursing facilities due to diabetes. This difficult placement can occur for many reasons, but predominately revolve around an inability to self-inject or the need for sliding scale dosages which they themselves cannot correctly administer. Sometimes these issues are further complicated by dementia or other behavioral concerns.

GOLDEN POND has made the decision to create a program to service these adults in an assisted living environment through a partnership with a provider of nursing services. The intent of this Diabetes Management Program is to provide a home environment to individuals who are unable to manage their diabetes care and insulin regimen, but who otherwise are candidates for assisted living placement, whether that would be in a traditional setting or in either of our special care units. Through our programming and our partnerships, we can provide both a high quality living situation as well as a safe environment for these individuals.
